Pentapeptide-18, commonly known as Leuphasyl, is a synthetic peptide composed of five amino acids (H-Tyr-Ala-Gly-Phe-Leu-OH). Scientists developed this peptide to mimic the body’s natural mechanisms that suppress muscle contractions. As a result, Pentapeptide-18 may help reduce the formation and depth of wrinkles along the epidermal surface.

Researchers hypothesize that Pentapeptide-18 interacts with neuroreceptors in the skin. More specifically, it may inhibit the release of acetylcholine, a neurotransmitter responsible for triggering muscle contractions. By reducing acetylcholine activity, Pentapeptide-18 may help relax underlying facial muscles, which can soften expression lines.

In addition to its neuromodulating properties, Pentapeptide-18 may support overall skin structure. Studies suggest that it may stimulate collagen and elastin production, two essential proteins within the skin’s extracellular matrix. Since levels of these proteins naturally decline with age, supporting their synthesis may improve skin elasticity and resilience.

Research and Clinical Insights

Pentapeptide-18 and Wrinkle Reduction

Clinical research has explored the potential of Pentapeptide-18 to reduce wrinkle depth and length, particularly along the stratum corneum. In a 2014 study, participants with moderate to severe wrinkles applied a Pentapeptide-18 formulation twice daily for 28 days. Using digital imaging and silicone replicas, researchers observed an average wrinkle depth reduction of 11.31%.

Importantly, investigators noted that while Pentapeptide-18 appears less potent than botulinum toxin, it may offer wrinkle attenuation without the associated side effects. Therefore, it may serve as a gentler alternative for cosmetic applications.

Further studies reinforce these findings. In randomized, double-blind, placebo-controlled trials, researchers reported a 2% to 9% statistically significant reduction in wrinkle appearance. Additionally, participants showed improved skin smoothness after four weeks, based on imaging analysis, dermatological assessments, and self-evaluations.


Pentapeptide-18 and Fine Lines

Beyond deeper wrinkles, this peptide may also reduce fine lines. Researchers suggest that it may decrease glutamate release by approximately 11% after two months of consistent use. Since glutamate acts as an excitatory neurotransmitter involved in muscle activity, lowering its release may reduce subdermal muscle contractions.

Furthermore, this peptide may also suppress acetylcholine secretion within the synaptic cleft. This proposed mechanism resembles the action of enkephalins, endogenous opioid peptides that regulate neurotransmitter release. By potentially modulating calcium influx into presynaptic neurons, Pentapeptide-18 may limit neurotransmitter exocytosis. Consequently, reduced calcium availability may lead to decreased acetylcholine release and diminished muscle contraction beneath the skin.


Pentapeptide-18 and Melanogenesis

Recent research has also explored modified Pentapeptide-18 derivatives and their potential impact on melanogenesis. In a 2020 study, scientists examined D-tyrosine–containing Pentapeptide-18 analogs using melanoma MNT-1 cells. These modifications involved replacing or repositioning tyrosine residues within the peptide sequence.

The findings suggest that peptide structure and amino acid orientation may influence melanin synthesis. Notably, this peptide analogs containing C-terminal D-tyrosine showed a potential reduction in melanogenesis. Researchers hypothesize that D-tyrosine may alter enzyme or receptor interactions involved in melanin production, thereby reducing pigment formation.
